DURING your first appointment…
Answer five quick questions about your symptoms, health history, and goals. No lengthy paperwork, no waiting rooms. Our brief intake form gives your provider the context they need before your visit.
Then choose your consultation type — select the appointment format that works best for your schedule and comfort level.
Your 30-minute virtual consultation connects you one-on-one with a licensed healthcare provider — via secure video from wherever you are. This is a real medical visit, not a chatbot or questionnaire-based service.
During your visit, your provider will:
- Review your intake responses and health history
- Ask follow-up questions to fully understand your concerns
- Order laboratory testing if clinically indicated
- Discuss treatment options appropriate for your individual situation
- Answer any questions you have about your care
A medical evaluation is required to determine whether any treatment is appropriate for you. Not all patients will be candidates for all therapies.
If treatment is clinically appropriate, your provider will prescribe a plan tailored to your individual needs. Medications are dispensed through our partner pharmacy — Mills Pharmacy & Apothecary in Birmingham, Michigan, a PCAB-accredited compounding pharmacy — and shipped directly to your door in discreet, secure packaging.

WHAT TO EXPECT AFTER YOUR FIRST APPOINTMENT
Your care doesn’t stop after your first visit. Your Formula provides ongoing support designed to help you stay on track, adjust your treatment when needed, and continue working toward your health and wellness goals.
Ongoing Provider Visits
You’ll have monthly visits with a licensed healthcare provider, such as a physician, nurse practitioner, or physician assistant. During these visits, your provider may order and review laboratory testing, monitor your progress, and prescribe or adjust medications as appropriate.
Nursing Support
When appropriate, you may choose a 10-minute visit with our nursing team for ongoing health management and medication refills under established provider orders.
Fitness & Bodybuilding Coaching
Take your goals further with personalized fitness or bodybuilding coaching from our IFBB specialist.
Nutrition Coaching
Work with our nutrition specialist for individualized guidance designed to support your health, fitness, and wellness goals.
Stay Connected Through the Elation Patient Portal

No. Your Formula does not accept or require insurance. Pricing is straightforward and self-pay.
Not necessarily. Laboratory testing is ordered only when clinically indicated based on your provider’s evaluation. If labs are needed, we have an established account with Quest Diagnostics for convenient, reliable access.
We provide virtual care to residents of Michigan and Florida. All visits are conducted online.
Most patients are seen same day or next day after completing their intake. We offer flexible appointment availability including evenings and weekends — because your health shouldn’t have to wait for a Monday morning opening.
